Job Details

• Weekly Estimated Pay: $2,490 - $2,960
• Hours Per Week: 40
• Shift Schedule: 8:00 AM – 4:30 PM, Monday to Friday
• Shift Duration: 8 Hour Days
• Shifts Per Week: 5
• Contract Length: 13 weeks
• Coverage Area: Marshall and Jackson Counties, Alabama (up to 40 miles from office)
• On Call/Weekend Requirement: Backup call one weekend per month and one night per week
• Dress Code: Royal blue or black scrubs
• Travel: Mileage reimbursed between patient visits

Job Requirements (RN Case Manager / Home Hospice RN)

• At least 1 year of RN Case Manager experience in home hospice care or home health case management
• Associate's degree or higher in Nursing (RN)
• Experience with Home Care Home Base Pointcare documentation (homecare clinical documentation systems)
• Ability to work independently in the field and manage home hospice patients
• Strong skills in admissions, recertifications, and discharge/death documentation
• Comfortable providing care in diverse home environments across all socio-economic levels
• Previous LHC group travel nurse experience is a plus

Responsibilities (Homecare / Hospice / Case Management)

• Manage hospice patients across Marshall and Jackson Counties, ensuring high-quality, patient-centered care
• Conduct admission visits, recertifications, and death/discharge documentation accurately and timely
• See up to six patients per day while maintaining timely clinical documentation
• Participate in IDG (Interdisciplinary Group) care team meetings every other Wednesday at the office
• Order patient medications and supplies as needed to support home hospice care
• Take backup call one weekend/month and one night/week using the Total Triage App
• Prepare thorough notes and documentation for care team meetings and clinical handoffs
• Collaborate with the hospice team while exercising autonomy in the field as a travel RN case manager
