Job Details — Home Health RN / Travel Nursing

• Weekly Estimated Pay: $2,490 - $2,960
• Position Type: Travel Contract / Travel Nurse — Home Health RN Case Manager
• Hours per Week: 40
• Shift Schedule: Monday - Friday, 8:00am - 4:30pm
• Shift Duration: 8-hour days
• Contract Length: 13 weeks
• On Call / Weekend Requirement: None
• Coverage Areas: Jefferson County & Blount County (Oneonta, AL area — zip code 35121)
• Mileage Reimbursement: Yes, between visits (from first patient to last patient)

Required Qualifications — RN Case Manager / Homecare

• Active Registered Nurse (RN) license (Alabama license or Compact RN eligible preferred)
• Minimum 1 year of nursing experience; home health or case management experience preferred
• BLS certification from American Heart Association required
• Experience with HCHB (Homecare Homebase) or PointClickCare / other EHR systems is a plus
• Strong assessment, clinical documentation, and care coordination skills

Key Responsibilities — Home Health Case Management

• Provide skilled nursing case management and home visits for homecare patients across Oneonta, Jefferson, and Blount Counties
• Perform comprehensive patient assessments and develop individualized care plans
• Coordinate care and community services, including referrals, durable medical equipment, and home health resources
• Monitor patient progress, adjust plans of care, and perform ongoing care coordination and discharge planning
• Communicate effectively with patients, families, primary care providers, and interdisciplinary teams
• Document interventions, assessments, and outcomes accurately in the electronic health record (EHR)
