Job Details

- Shift Schedule: 8:00 AM – 4:30 PM, Monday through Friday

Job Requirements / Qualifications
- Active Registered Nurse (RN) license (state requirements apply)
- Minimum 1 year of nursing experience; homecare or home health experience preferred
- Strong patient assessment and documentation skills, including OASIS documentation
- Ability to work independently, manage a caseload, and provide case management in the home setting
- Reliable transportation for travel within the 50-mile coverage area
