Shift Details:

• Shifts will be 8-12 hours and candidates will be needed for full and part time.

Licensed Practical Nurse (LPN)  Duties & Responsibilities:

• LPN's observe, measure, and record clinical data, which is used by other health practitioners to make health care decisions.  

• LPN's can pass medications, complete wound care, administer vaccinations and execute medical regimens as prescribed by NY Licensed Physicians, Dentists, Nurse Practitioners, Physician Assist and Podiatrists.  

•  LPN's identify health problems or changes in a person's health status and determine whether immediate intervention is needed.  

• LPN's must report these problems or changes in a timely manner to an RN or directing practitioner.  

• LPN's are not legally allowed to take independent action to address health problems or changes. 

• Medication administration and tube feeding via gastrostomy or jejunal tube observation

• Provides personal care and maintains living areas, including making beds, changing linens, lifting and moving individuals, and safeguarding personal belongings.

Minimum Qualifications 

• Current, valid, and unrestricted NY license/certification in good standing, as required for the position.

• Candidates should be experienced with developmental disabilities, 

• Administration of medications/including Insulin; G-Tube/J-Tube feedings; Respiratory treatments, Tracheotomy care/Suctioning, etc.

• Minimum of one (1) year/s of recent experience as an LPN.

• Additional certifications may be required.

• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
