Home Health LPN/LVN Job Details

• Weekly Estimated Pay: $1,370–$1,440
• Specialty: Long-Term Care / Home Health
• Location: Arlington, Texas
• Start Date: October 5, 2026
• End Date: January 1, 2027
• Contract Duration: 13 weeks
• Nursing Schedule: 8-hour shifts, Monday–Friday, approximately 8:00 a.m.–4:30 p.m.
• Hours: 40 hours per week; 5 shifts per week
• On-Call Requirement: Shared rotation covering weekdays and weekends
• Patient Care Setting: Home health nursing in patients’ residences
• Travel Detail: Mileage between patient visits is reimbursed

Home Health LPN/LVN Job Requirements

• Active Texas Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license
• At least 1 year of professional nursing experience; home health nursing experience preferred
• Experience using Homecare Homebase (HCHB) strongly preferred
• High school diploma, GED, or equivalent
• Current professional certifications and licensure required for practice
• Ability to travel throughout the Arlington service area and provide skilled nursing care in patients’ homes
• Willingness to participate in a shared on-call rotation
• Navy blue scrubs required

Home Health Nursing Responsibilities

• Provide skilled nursing visits to patients in their homes
• Perform comprehensive nursing assessments and monitor patient condition and progress
• Deliver wound care and manage complex wound treatment plans
• Provide care involving wound vacuum (wound vac) systems, PICC lines, PleurX drains, and other specialized clinical needs
• Administer medications and treatments in accordance with established care plans and clinical standards
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs
• Document patient visits accurately and efficiently in Homecare Homebase (HCHB)
• Coordinate with providers, caregivers, and the interdisciplinary care team to promote continuity of care
• Respond to patient needs during assigned on-call coverage
• Manage a daily home health visit schedule while meeting established productivity expectations
