Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license eligible for practice in Massachusetts
• At least 1 year of certified home health or certified home care nursing experience
• At least 1 year of case management experience within a certified home health agency
• Home health nursing certification or demonstrated competency in home health practice
• Ability to travel independently to assigned patient cases
• Strong clinical judgment, communication, organization, and time-management skills
• Proficiency with timely electronic documentation and interdisciplinary communication

Home Health RN Responsibilities

• Perform comprehensive nursing assessments of patients in their home environments
• Develop, implement, and evaluate individualized plans of care
• Provide skilled nursing interventions and monitor patient progress
• Complete admission processes and ongoing home health case management activities
• Educate patients and families about diagnoses, medications, treatments, and self-care
• Coordinate care with healthcare providers and interdisciplinary team members
• Identify changes in patient condition and communicate concerns promptly
• Maintain thorough, accurate, and timely electronic clinical documentation
• Travel to assigned cases throughout the service area and participate in the scheduled weekend rotation
