• Monday - Friday, 8 am - 5 pm -- 9-hour shifts, 8 hours guaranteed
• 4 - 6 patients per shift
• Combination of in-clinic and community home visits required
• Adults over age 55 served through PACE program
• Geriatric experience required
• Experience working with interdisciplinary teams preferred
• Will consider 3 - 4 days per week schedule
