Job Requirements

• Active Licensed Practical Nurse (LPN) license
• At least 1 year of experience in home health, home care, or skilled nursing care
• Strong understanding of the State Nurse Practice Act and home health nursing standards
• Ability to work independently while collaborating with RNs, therapists, home health aides, and the interdisciplinary care team
• Valid driver’s license and reliable transportation for patient home visits
• Current credentials and onboarding requirements completed prior to start

Responsibilities

• Provide skilled nursing care to patients in the home setting under the direction of the RN Case Manager and Clinical Team Manager
• Deliver care according to established policies, procedures, and professional nursing standards
• Complete approximately 6 patient visits per day while maintaining quality, compassion, and patient safety
• Support patients through assessments, treatments, medication-related tasks, and other delegated nursing duties
• Collaborate with the interdisciplinary team to promote safe, effective, and individualized home health care
• Document patient care accurately and in a timely manner
• Maintain professionalism, dependability, and excellent communication with patients, families, and team members
