Registered Nurse Case Manager (RN) – Inpatient Case Management Contract

We are seeking a compassionate and organized Registered Nurse Case Manager for an on-site, 13-week contract assignment in Pompton Plains, New Jersey. This RN case management opportunity focuses on inpatient care coordination, discharge planning, transitions of care, and safe patient progression. You will help hospitalized patients and their families navigate care transitions, coordinate services, support safe discharge planning, and promote efficient progression through the continuum of care. • Schedule: Day shift, 8:00 a.m. – 4:00 p.m.
• Shifts per Week: 5
• Hours per Week: 37.5
• Shift Duration: 8 hours
• Work Setting: On-site inpatient hospital environment
• Weekly Estimated Pay: $1,833 – $2,036

Registered Nurse Case Manager Requirements

• Current Registered Nurse license eligible for practice in New Jersey
• Graduate of an accredited nursing program
• At least 1 year of professional nursing experience, including experience caring for hospitalized patients
• Inpatient hospital experience with discharge planning required
• Knowledge of case management principles, care coordination, utilization of services, and transition planning
• Understanding of applicable regulatory standards related to inpatient care and case management
• Excellent communication, negotiation, collaboration, and conflict-resolution skills
• Strong computer, clinical documentation, and data-management skills
• Case management certification or formal case management training is preferred

Inpatient Case Management Responsibilities

• Assess patients’ clinical, psychosocial, functional, and resource needs to develop individualized case management plans
• Collaborate with physicians, nurses, social workers, utilization review staff, patients, and caregivers to coordinate care
• Develop, document, and continually update safe and timely discharge and transition-of-care plans
• Identify barriers to discharge and recommend solutions that support appropriate length of stay and efficient patient progression
• Recognize high-risk patients and refer them to social services or other appropriate resources
• Coordinate transitions between acute care and post-acute settings, ensuring clinical needs and follow-up services are addressed
• Provide education and resources to patients, families, and the care team regarding care progression and available services
• Monitor issues involving access to post-acute services and communicate concerns to the interdisciplinary team
• Support readmission prevention efforts by assessing risk factors and helping implement appropriate interventions
• Identify potential quality or risk-management concerns and escalate them through the appropriate channels
