Job description
Join us as a Case Manager Registered Nurse (LCSW or RN) on an 13-week travel assignment at a reputable short-term acute care hospital in New London, Connecticut. This role offers a balanced schedule of 8-hour days, working every other weekend, in a vibrant hospital setting that handles med/surg, neurology, oncology, cardiac, and obstetric patients. You’ll have the chance to impact patient care through care coordination and discharge planning, all within a Level III trauma center environment. Epic charting experience and at least two years of acute hospital or home health experience are essential for success in this role. The hospital is part of Yale New Haven Health System and features a supportive team, free parking, and a chance to work with a diverse patient population that's always engaging—sometimes even a little fun!
